• Fill out with complete patient demographic information
• For COVID – 19 test type, please select ‘SARS – CoV – 2’
• Select the proper collection date for their event
• Click ‘CREATE E-REQ” at the bottom of the screen
• Print out E-REQ
• Bring the print-out with you to the collection site in order to be tested
